Inspection response for holes that passed and failed inspection

How can a form inspection be used to indicate that some holes have been inspected but failed, while others have passed? Is there a way to link this to the field activity? This will highlight where a hole has been visited but failed the inspection. For example, sign off failed due to works required on the pad.
